In our November Market Insights report, Leapfrog analyzes patient safety performance for the five largest health systems eligible to participate in the Hospital Safety Grade, using CMS PSI 90 scores—the most heavily weighted measure in the Safety Grade. PSI 90 summarizes performance across ten Patient Safety Indicators, and this month's findings explore variations in the most heavily weighted measures: pressure ulcers, postoperative respiratory failure, perioperative pulmonary embolism, and postoperative sepsis. While the results emphasize that overall PSI 90 performance varies, each system has areas of excellence where targeted clinical practices, consistent procols, and strong system-level oversight can drive measurable reductions in preventable harm and advance patient safety.
